Welcome to the HOPE Infant Family Support Program
The HOPE Infant Family Support Program partners with families of infants and toddlers (birth to 3 years old) who have disabilities. Our mission is to provide individualized, innovative, and culturally responsive services that reflect each family's unique strengths, priorities, and needs.
We offer family-centered, coordinated early intervention services that promote children's development and full participation within their families and communities. Services are guided by the Individualized Family Support Plan (IFSP), developed collaboratively by the family, HOPE staff, and representatives from other California Early Start agencies.
Each family is supported by:
-
A Service Coordinator, who helps navigate the system and ensures services are in place.
-
A Primary Service Provider, who serve as consistent points of contact and provides coaching and direct interaction tailored to the child’s and family’s goals.
Services are delivered in the child’s natural environment, such as the home or community settings, and may also include input from specialized consultants when needed. Our team works in close partnership with families to ensure services are meaningful, respectful, and effective.
