Underwriting Survey
It is time, once again, for the annual Property & Liability Underwriting Survey. The JPA will be collecting your
district underwriting information electronically again this year.
When you click on the link provided, you will be directed to the 2024‐2025 Underwriting Survey in Microsoft
Forms. Please note that the Form cannot save your data, so it must be completed in one sitting. To assist you with
this process, we have attached a link to a .pdf version of the 2024‐2025 Underwriting Survey posted on our
website. Please use this tool and then complete the Form when you have all of your data.
We ask that you please complete this on‐line survey no later than January 31, 2024.
Completing/updating this survey helps us properly evaluate coverage. Please click on the following link to

Exhibits 1, 2, 3, 4 & 5, if applicable. District spreadsheets are acceptable for all exposures other than Property.
Your Property Valuation worksheet, sent to you in September 2023, is in the format required by our broker. No
other formats will be accepted. If you have not already done so, please update this worksheet and send it back to
us with the other exhibits.
Your participation in providing required data will assist our broker in developing our FY 2024‐2025 insurance
