Complete and collect:
- SDCOE Program Clear Application
- CTC Credential Application (PDF)
- Note: Do NOT forward date the 41-4.
- Either wet signature OR approved Adobe and DocuSign electronic signatures are accepted.
- Temporary County Certificate Application
- Only for teachers at public school districts, contact your employer to obtain the correct form for your county of employment
- Official Transcripts
- Not required for holders of a Clear Single or Multiple Subject Credential
- If you completed either of these courses through SDCOE, please order official transcripts from our education partner, University of Massachusetts Global.
- Health Education (The commission does not accept SDCOE certificates of completion)
- U.S. Constitution (The commission does not accept SDCOE certificates of completion.)
- If you completed either of these courses through SDCOE, please order official transcripts from our education partner, University of Massachusetts Global.
- Not required for holders of a Clear Single or Multiple Subject Credential
- Copy of CPR Card or Certificate (showing Infant, Child, and Adult CPR skills)
- Verification of Teaching Experience (PDF)
- The $100 application fee to the CTC will be paid online through the educator profile by the candidate, following the submission of a formal online recommendation for the credential by the program. (4.25.2024)
Step 3
Turn in all documentation to your employer's Human Resources Credential Technician. They will mail the packet to our Credentials Office for processing.
DO NOT SCAN AND EMAIL APPLICATIONS. SDCOE CANNOT ACCEPT SCANNED APPLICATIONS DUE TO THE SENSITIVE PERSONAL INFORMATION.

- Adult Education
Adult Education programs offer their communities both state-supported general education courses and community education fee-based classes.
- Career Technical Education
Career Technical Education is one of the fastest-growing areas of education. These courses prepare students, from middle school to adult, for specific trades in 15 industry sectors. Teachers pursuing a CTE credential come directly from those industry areas.
- Special Subjects
The Special Subjects Credential authorizes teaching in programs such as Reserve Officer Training Corps (ROTC), Driver Education, and Aviation. These programs are not offered in all districts, and credential requirements and necessary experience are very specific.
