Student List: Save a copy of one of these two versions of the Excel Student List to your computer. Fill out the list with all relevant information. The more information you put in the comments section, the better able we are to place students. Email the completed Student List at least three weeks prior to your scheduled camp date.

Student Health Form

Print enough copies of the Student Health form double-sided on pink paper for each participating student. Send home to students’ parents/guardians to be completed and signed. Once completed and returned to school, mail them to Cuyamaca Outdoor School at least three weeks prior to your scheduled camp date.

Medication Authorization

Print enough copies of the Student Health form double-sided on white paper for each participating student who will be bringing prescription medication or their own non-prescription medication/vitamins/supplements. Send home to be completed and signed by both a parent/guardian and a physician. Here are the Instructions for Completing Medical Authorization Form.  Bring this form, along with the accompanying medication/s, to Cuyamaca Outdoor School and deliver to the Health Center upon arrival.

Special Dietary Requests/Needs

Any student who requires special meals and/or meal accommodations (except vegetarians and vegans) must complete the Medical Statement to Request Special Meals and/or Accommodations form. Parents may contact our Child Nutrition Supervisor directly to discuss meal plans

Packing List

Please review the packing list with your students before sending it home. There is no cell phone coverage at Cuyamaca, so students should not bring a cell phone.

Work with the camp coordinator and your colleagues to complete the Student List that will be sent to camp four Mondays before your trip. Include information in the comments section that you think may be helpful for student placement in the cabin and trail groups, such as students who should be kept together, students who should be separated from each other, and any other information that you think will help our staff ensure a successful experience for your students.

The My Journey Through 6th Grade Camp video, created by a Cuyamaca Outdoor School alum, is an excellent tool to help your students prepare for their week at Cuyamaca Outdoor School.
