Referral Process

Students are required to register in their district of residence and meet eligibility for an Individualized Educational Program (IEP) and have a qualifying hearing loss.

The student’s district of residence must hold an IEP meeting to consider placement options. If the district of residence offers Davila Day School as the appropriate placement, a school site tour can be scheduled. To schedule a school tour, please contact the main office.

Enrollment Procedures

Students can enroll at Davila Day School after the district of residence IEP team has agreed on placement at the school. Once placement occurs, parents must complete the following forms to enroll their child, and contact the principal, Ms. Cheryl Kaler, at cheryl.kaler@sdcoe.net to arrange a start date.

• Child's immunization records
• Registration Forms (required, can be found on the Davila Day School website, under school information).
• Photo identification (e.g., driver's license)
• Proof of residence, two forms, such as:
  • Title or escrow papers, mortgage book or statement, or homeowner’s association fees statement
  • Lease agreement or rental contract with landlord’s name, address and telephone number and current rent receipt
  • Letter on apartment complex or mobile home park letterhead, signed by the landlord, stating that the parent/guardian/caregiver lives there (mandatory with 2nd Family Affidavit)
  • Utility bill: gas and electric, water, cable TV, sewer, or waste disposal bill (telephone bills are NOT acceptable; past due or disconnection notices are NOT acceptable)
  • Verification of Social Services (SSI, AFDC, Medi-Cal)
