Educational Audiology and Deaf/Hard-of-Hearing Itinerant Services

From I-5 North:

Take H Street, exit 8A. Turn right onto H Street. Go past Broadway, make first left onto Smith Ave. The parking lot is on the right before G Street. Parking is available in the Smith Ave parking lot in front of Davila Day School or on the street. The entrance to Davila Day School is at the north end of the parking lot. Ring the doorbell at the PURPLE GATE.

From I-5 South:

From 805 North:

From 805 North: take H street, exit 7. Turn left onto H street. Go several lights down, make first right after 5th ave onto Smith Ave. Parking lot is on the right before G Street. Parking is available in the Smith Ave parking lot in front of Davila Day School or on the street. The entrance to Davila Day School is at the north end of the parking lot.
Ring the doorbell at the PURPLE GATE.

From 805 South:

From 805 South: take the first H street exit (H street West), exit 7B. Merge onto H street. Go several lights down, make first right after 5th ave onto Smith Ave. Parking lot is on the right before G Street. Parking is available in the Smith Ave parking lot in front of Davila Day School or on the street. The entrance to Davila Day School is at the north end of the parking lot. Ring the doorbell at the PURPLE GATE.

These rating scales are only to be completed as requested by your South County SELPA Audiologist. 

Three types of hearing loss are outlined below: sensorineural, conductive, and mixed. 
Hearing levels are defined using descriptors (eg. mild, severe), not using percentages. 
Example: Mild to moderately - severe sensorineural hearing loss. 

Sensorineural	Conductive	Mixed
There is a problem with the hearing organ (cochlea) and/or auditory nerve; the outer and middle ear portions are intact.	There is a problem in the ear canal and/or middle ear space; the hearing organ (cochlea) is still intact.	There is a problem with the hearing organ/auditory nerve as well as an ear canal/middle ear component.
