To enroll parents must bring:
- Child's immunization records
- Photo identification (e.g., driver's license)
- Proof of residence, two forms, such as:
- Title or escrow papers, mortgage book or statement, or homeowner’s association fees statement
- Lease agreement or rental contract with landlord’s name, address and telephone number and current rent receipt
- Letter on apartment complex or mobile home park letterhead, signed by the landlord, stating that the parent/guardian/caregiver lives there (mandatory with 2nd Family Affidavit)
- Utility bill: gas and electric, water, cable TV, sewer, or waste disposal bill (telephone bills are NOT acceptable; past due or disconnection notices are NOT acceptable)
- Verification of Social Services (SSI, AFDC, Medi-Cal)
